Forever Fern Wine Bottle Tag

I love making these projects and this  Forever Fern Wine Bottle Tag is no exception.  These are quick to make and your host will love that you took time to make a unique gift out of a bottle of wine.  You can even make several of these and just have them handy.  Here is the sketch to follow:

You can alter the length and width as you see fit.  I like these dimensions and use them often.  You can even write a quick note on the back side of the forever fern wine bottle tag.
